DeSantis Family Practice is proud to announce that our primary care practice has earned a Level II Designation as a Patient-Centered Medical Home.  Being a Patient-Centered Medical Home means we are committed to your total well-being by providing comprehensive, continuous medical care.  It means building a lasting partnership with you so that together, we can care for your health today and help prevent illnesses tomorrow.

What it means to receive the Patient-Centered Medical Home (PCMH) Recognition

The Patient-Centered Medical Home is a model of care that puts patients at the forefront. PCMHs build better relationships between people and their clinical care teams.  Research shows that they improve quality, the patient experience and staff satisfaction, while reducing health care costs.

NCQA’s Patient-Centered Medical Home Recognition Program is the most widely adopted Patient-Centered Medical Home evaluation program in the country.  More than 12,000 practices (with more than 60,000 clinicians) are recognized by NCQA.  And more than 100 payers support NCQA recognition through financial incentives or coaching.

Practices that earn recognition through NCQA have made commitments to quality improvement within their practices and adopted a patient-centered approach to care that results in happier and healthier patients.

The NCQA is a private, non-profit organization whose goal is to improve the healthcare system in the United States. NCQA ACO Accreditation Level 2 are for organizations demonstrating well-established capabilities outlined in the standards to meet the triple aim of better patient experience, better health and lower per-capita cost. The length of status is 3 years.

Becoming a patient-centered medical home is just one of the many ways we are striving to give our patients the best possible health care.
