    Below are questions we frequently receive from patients -- please click on the down arrow to the left of the questions to view the answers.

    What should I bring to my appointment?

    Please bring the following to each visit:

    • Photo ID
    • Your current health insurance card
    • Your current address and phone number
    • A list of the medications you are currently taking or the medication bottles of the prescriptions you currently have
    • Any forms that you need your provider to complete for you
    Can I schedule a same-day appointment?

    Some same-day appointments are available if you have an illness or injury that needs immediate medical attention. If possible, we will make your appointment with your primary medical provider. However, if your primary medical provider is unavailable, another provider will see you.

    What rights do I have that relate to my health care?

    DeSantis Family Practice follows the promises we make to our patients in our Patient Bill of Rights. This means we respect your right to be treated respectfully and courteously. We also care for you in ways that follow all federal and state laws that establish patient rights. This includes the confidentiality of patient information as required by the Health Insurance Portability and Accountability Act (HIPAA). You may request a copy of our patient privacy policy at any time.

    How will I receive my test results?

    We will contact you by phone with any significant abnormal results. You may need to wait up to four weeks to receive some test results. Sign up for Patient Portal and we can send your labs through a secure message. Otherwise, you may pick up a copy of your labs in person.

    What if I need a prescription refilled?

    When you need a refill of one of your current, regular medications, please contact your pharmacy. They will then contact us to authorize a refill. It can take two to three days for your medication to be refilled. Please call your pharmacy to ask for a refill while you still have enough medication for four more days. You may also request refills during clinic visits.

    Can you refer me to another provider?

    We are part of a network of health-care providers. If you wish to be referred to a provider not in this network, we may be able to refer you. Please feel free to contact us with your questions.
